Payment Methods

• We accept some insurance plans and will submit claims for those on your behalf. If your insurance plan is not one of those accepted, we will provide you the information to submit your own claim.
• We accept cash, Visa, Mastercard, Discover, American Express, and bank debit cards. No personal checks are accepted. 
• Additionally, we offer a payment plan through CareCredit which offers interest-free financing for up to 6 months. See www.carecredit.com or call our office for details.

Requesting an Appointment

Thank you for choosing Special Anesthesia Services. Our CRNAs work with over 100 dentists in Kansas, including many in the Wichita area, providing sedation services to children and adults in the dental office setting. 

Please complete the Appointment Request Form. All boxes are required in order to submit the form.

Next Steps

1. After the Appointment Request Form has been completed, Special Anesthesia Services will process the information. 
2. You will receive a link with the required Sedation Paperwork through a text and/or email. Please complete all required information and upload a copy of the patient’s insurance card when prompted. This information is REQUIRED in order to proceed with the sedation appointment. 
3. Once the Sedation Paperwork packet has been completed and submitted, a staff member from Special Anesthesia Services will reach out regarding medical clearance and finances/deposit for the upcoming appointment (if needed). 
4. Our office will call 1 week prior to the scheduled sedation appointment to complete NPO/Pre-Op instructions for the upcoming appointment.
